If you plan to cancel your registration as a regulated member in Alberta due to retirement, moving out of the province or outside of Canada, sabbatical, maternity leave or post-graduate studies, you will need to complete the attached form and declaration. Until this signed form is received at the office of the College of Dental Surgeons of Alberta (CDSA), you are considered a member with the same responsibilities and obligations of meeting continuing competency requirements, maintaining malpractice insurance and payment of annual dues. As part of your cancellation you will need to cancel your Professional Liability Insurance administered by Navacord Insurance Services. You can contact Navacord directly at dentist@lloydsadd.com

Notify the College of Dental Surgeons of Alberta
- Complete the Cancellation of Practice Permit form within the Registrant Portal.

Inform your patients
If you plan to retire and you own your clinic, you must notify your patients and provide details about the future of the dental practice including:
- whether the practice is being sold to a new dentist;
- what will happen to their dental records;
- who will continue treatment for patients with planned or partially completed treatment or who have conditions that need monitoring.
If you are a member who is planning to cancel your membership and you do not own a practice, you will need to refer to the contract you have signed with your hiring dentist.
Cancel Your Professional Liability Insurance
As part of your cancellation you will need to cancel your Professional Liability Insurance administered by Navacord Insurance Services. You can contact Navacord directly at dentist@lloydsadd.com. Registered members are qualified for a partial refund for the insurance premium included in their original permit fees.
Update your Contact Information
The CDSA requires your up-to-date contact information. Anytime you have changes to your contact information (Name, Address, Phone Number, email), please update your contact information in the Registrant Portal.
Registrant PortalReturn Prescription Pads
Return all partially or fully unused triplicate prescription pads to the Tracked Prescription Program with the College of Physicians and Surgeons of Alberta, to be destroyed. Any other prescription pads should be destroyed by confidential shredding.
Medications
You will need to properly dispose of any sedatives, antibiotics, painkillers and emergency drug kits in your office. Please visit the Alberta Pharmacists’ Association website for the ENVIRx Disposal Program, which contains a listing of pharmacy locations and contact information that provide this service.
Prepare your Professional Corporation
In Alberta, only a regulated member can hold a named professional corporation. The CDSA is required by law to notify the Registrar of Corporations once a member holding a named professional corporation is no longer registered with the CDSA. You will have a 90 day grace period to either amend your named professional corporation to a holding or numbered company or have the corporation decommissioned. Please consult with your lawyer or accountant for advice. Please refer to the Professional Corporations Information Package for more details.
Store your Dental Records
Dentists in Alberta are required to keep dental records a minimum of 10 years from the date of last treatment of a patient, and if the patient is 18 years of age or under, records must be kept 10 years from the date the patient turns 18. Records must be transferred to another regulated member of the CDSA with an agreement that the other member will act as a custodian of those records. They can be stored at the custodian’s dental practice or in secure off-site storage.
Advise CDSA of Sale or Disposal of Dental X-Ray Equipment
Employers, Responsible Dentists and owners of dental x-ray equipment have obligations to notify the CDSA Radiation Program in writing within 30 days after selling, leasing, transferring, lending, assembling, or installing designated radiation equipment.
The following information needs to be provided:
- the regulated member’s name and address
- the manufacturer, model, serial number and, if available, date of manufacture of the designated radiation equipment
- the date on which the ownership or possession of the designated radiation equipment changed or on which designated radiation equipment was assembled or installed

If you are an Accredited Dental Facility
You will need to declare whether you are an accredited facility owner or dental operator.If you are, please advise the CDSA 30 days prior to cancelling your registration as a Dentist in Alberta.
Dental Surgical Facility Accreditation Regulation, Alberta Regulation 250/2002
Section 4 The accreditation requirements for dental surgical facilities are compliant with the standards for dental surgical facilities approved and published by the council.
Standards of Practice: Minimal and Moderate Sedation Deep Sedation and General Anaesthesia in a Non-Hospital Dental Practice
Section A1 A Dental Surgical Facility (DSF) must have a Dental Operator. A Dental Operator is a regulated member who is an Operator and the registered owner or the person who has the apparent care and control of the DSF.
Section C1.2 A Dental Operator must:
- Notify the CDSA in writing and in anticipation of or in advance of any intended change of the Dental Operator of the Accredited Dental Surgical Facility.
- Section C1.3 A Dental Operator must, in anticipation of or in advance of any of the following changes with respect to an Accredited Dental Surgical Facility, advise the CDSA in writing of:
- Any change of ownership of the Accredited Dental Surgical Facility.
